Children’s Community Nursing
The Children's Community Nursing teams provide a service to children aged 0-19 years. They provide short term nursing support such as wound care, as well as support for long term conditions and specialist training for carers and patients.
The team work with the children and their families to enable early discharge from hospital and the prevention of admissions.
They can be lead professionals for families who have a child with highly complex needs and also provide palliative care.
The team consists of:
- Specialist Community Children's Nurses who have developed an expertise in children with complex and lifelong health problems
- Children's Community Nurses
- Specialist Continence Nurse
- Clinical lead for Continence
- Health Care practitioners
