Help and support
The help and support available by home assessment and treatment includes:
- Social contact and someone to talk to, frequent visits and telephone contact.
- 24 hour 7 day a week access to help when needed.
- Ability to stay in a place of safety and sanctuary.
- Practical support, e.g. help with shopping or cooking meals.
- Help with medication.
- Continuous assessment.
- Therapeutic support, e.g. Talking Therapies.
Working with carers
To improve patient recovery and our quality of service it is vital that we working closely with carers. We aim to:
- Work closely with carers and provide 24 hours a day support.
- Involve carers in the care planning process.
- Provide an opportunity for carers to discuss their issues in relation to their role as carer.
- Ensure that a Carer's Assessment is completed.
- Support carers in developing coping strategies and other skills to support their role.
- Provide copies of the service user's home treatment care plan if requested with the service user's consent.
Hospital admission
Your recovery will be improved by being in your own environment with the support of family and friends so we try to keep hospital stays as short as possible. If you need to be admitted to a hospital you will receive all the information about where you are staying and you will be given all the contact details for your family, friends and carers.
Ongoing care
The important thing to know is that we will be involved in your care until your crisis has been resolved. If you need to be, we will make sure that you are linked into ongoing care by referring you to other mental health or social care services. Leading up to you being discharged from CRHT we will give you advice and information on what to do if you or your carers think you are beginning to feel unwell again and your GP will be given details of your care and treatment with our team.
You will always be able to take part in your own care as you will be invited to be a full partner in your care programme. This means you can:
- Attend meetings with nurses, doctors, support workers and housing officers.
- Attend care review and planning meetings to be fully involved in your recovery and ongoing care.
- Choose someone from your family or care team or an independent representative to support or stand for you at meetings.
- Choose to arrange more intimate discussions with ourselves and the professionals involved in your care.
