Bay Health & Care Partners welcome publication of the sustainability and transformation plan

Bay Health and Care Partners – the 11 organisations working together to deliver the Better Care Together strategy – have welcomed the publication of the Lancashire and south Cumbria Sustainability and Transformation Plan (STP). 

Better Care Together is the core delivery plan for south Cumbria and north Lancashire – a journey of transformation which began in autumn 2012. 

Health and care organisations across the footprint of the STP face similar challenges – for example delivering integrated care; meeting national commitments such as more focus on self-care and prevention, and long-term plans built around the needs of local areas and populations. 

Andrew Bennett, Chief Officer, NHS Lancashire North Clinical Commissioning Group said:

“By working together Bay Health and Care Partners are already producing solutions for sustainable health and care services that meet the needs of the local population, now and well into the future.” 

For example, locally we have seen: 

  • Health and care professionals working across traditional boundaries to better join up primary, community and secondary care services 
  • Health and care professionals working in partnership with local communities to keep people fit and well. 
  • Staff helping people manage their own condition-whether at home or in their local community 
  • The strengthening of a new out of hospital model of care with multidisciplinary, integrated care teams working in integrated care communities across Morecambe Bay, supporting long term, chronic conditions, frail elderly and others at risk of hospital admission. 

Aaron Cummins, Deputy Chief Executive, University Hospitals of Morecambe Bay NHS Foundation Trust said, 

“Over the past two years we have been working together as a partnership to transform healthcare services in Morecambe Bay. With the publication of the Healthier Lancashire and South Cumbria Sustainability and Transformation Plan, which includes our Better Care Together delivery plans, we look forward to working even more closely with our colleagues across the wider Lancashire health system to improve service quality and enhance population health and wellbeing.”
