Early Supported Stroke Discharge (ESSD) Teams
What we do
Provide specialist assessment and early, intensive rehabilitation in the community, for people who have had a new stroke following an acute episode of care
Who we are
The teams include:
- Occupational Therapists
- Speech and Language Therapists
- Rehabilitation Assistants
We also have close links with the stroke consultants, specialist nursing teams, social services and 3rd sector organisations such as the Stroke Association.
The teams help stroke survivors to make a smooth and early transition from hospital back to their own community in order to maximize the potential for recovery. Once we have received the referral from the hospital, we work closely with the stroke survivor, their family, carers and the multi-disciplinary team on the stroke ward to ensure therapy and care continues at home.
Once home, there is no set time limit for therapy. The duration and intensity of therapy with the teams are based on the individual’s need. However the aim is to provide the appropriate tools, strategies, resources for the stroke survivor to ‘take charge’ of their own journey to recovery and be as independent as possible in all aspects of their lives. As part of this process, the teams provide regular progress and planning meetings to ensure the appropriate services and support are in place on discharge from the service. The intensity of the teams’ input will gradually decrease as the stroke survivor progresses.
Every stroke survivor receives a personalized rehabilitation pack which is set up in collaboration with the stroke survivor and includes their goals, therapeutic activities, rehabilitation-focused care plans and information/resources on all aspects of stroke and stroke recovery.

New developments in stroke services for Cumbria
We currently have ESSD teams set up in Carlisle/Brampton and Penrith localities. There are exciting plans for a new hyper acute stroke unit at the Cumberland Infirmary in Carlisle as well as a new ESSD team for Whitehaven and surrounding areas. This will mean improved pathways for people in Cumbria who have suffered a stroke and ensure they get the best possible outcomes and recovery. The aim is for the enhanced stroke services to be in place by early 2019. We’ll keep you updated!
