Better Care Together

Bay Health & Care Partners
Better Care Together is a strategy for the future of healthcare for communities in South Cumbria and North Lancashire. It is being led by 11 organisations working in partnership, known as Bay Health & Care Partners, including Cumbria Partnership NHS Foundation Trust. 
 
The strategy aims to implement new ways of working, meaning more people in our community will be cared for closer to, or in, their own homes, and fewer people will need to go to a hospital for specialist treatment. This will be done by:
  • Organisations working together as one system.
  • Meeting the needs of the local population now and well into the future.
  • Enabling communities to be as healthy as possible.
  • Working closely with local communities and supporting people to manage their conditions.
  • Making sure that health and care services in this region are the best they can be.
Progress to date
Great progress has already been made, from a reduction in hospital referrals, through to patients not needing to travel as far through the use of ‘tele-health’ video technology. You can read more about the progress to date by scrolling down to our news stories below. 
 
A 12 month evaluation is now underway and patients, staff and the public are encouraged to get involved
